Training was recognized as a “quality standard” 20 years ago 1 and quality improvement is now linked to the assessment of doctors in training2 and to the outcomes of undergraduate medical education.3 In the UK, the expectation of doctors in the first two years after training is graduation. Set out by the UK Foundation Year curriculum, which states that FY2 doctors must make a significant contribution to at least one quality improvement project and report on their work in their electronic portfolio.4

Two systematic reviews found that improvements in the quality of training and patient safety for trainees lead to consistent changes in clinical practice. Subsequently, empirical reviews7 and systematic reviews8 have focused specifically on characteristics of quality improvement training that are associated with positive, sustained effects on patient care outcomes and improved system performance.

However, previous articles in this series have raised concerns that trainees in the UK are short-circuited, have little time or support, and may feel that they do not have the authority to convince their peers that problems need to be addressed.910 This article uses evidence. FY2 is a cycle of three releases. The British Foundation program requires three hours of non-clinical professional development time per week in FY2. The purpose of this time will include preparing for specific applications and developing skills in quality improvement, teaching and leadership.

In the early years of our program, we focused on emergency care (acute medicine, pain medicine, and emergency medicine) because the volume of patients enabled quality improvement projects in the short time available, and this was a priority area in the program. Patient Safety Scotland which has built capacity to engage frontline staff in improvement practices. Over eight years we have built capacity for quality improvement projects in the various services in which FY2 doctors work.

During the first three years of the program, several FY2 quality improvement projects focused on issues identified by foundation year physicians. Some of these projects led to structural changes that have persisted since 1314. However, it is often difficult to sustain changes that increase workload for other team members. We learned from 15 quality improvement projects that include them. Identified the problem. And clinical teams enable them to design and test solutions, 16 that can be sustained
