This is the time when many companies give their employees a window to review any changes to their health insurance plans, including how much more it will cost. It’s also the time for Obamacare health insurance plans to open on the federal government’s Healthcare.gov website for the 28 states that use it, including Florida.

Health insurance is a big expense for most people. It comes out of your salary or bank account every month. Along with this come direct expenses, such as medical fees and deductibles. Like all insurance, for this money, you buy to transfer some of the financial risk of paying for escalating health care costs to an insurance company.

In Florida, the largest is Florida Blue. About 2 in 5 Floridians with individual health insurance get it through Florida Blue. One in three with insurance through a small employee gets it through Florida Blue. More than half of the major employers use Florida Blue.

Spoke with Penny Shafer, the company’s South Florida leader about the Affordable Care Act, the employer-based health insurance market, rates, costs and how it all affects our regional economy.

What you see tries to balance the affordability of the program itself with a percentage of membership costs. This is true across the board. We need to push people to become better consumers of health care dollars — whether it’s what their employer spends, what they spend out of pocket, or what their coverage would allow, and to become better users of those health care dollars.

At the macro level, we were at about 17 to 18 percent of GDP (spent on) health care before we passed the Affordable Care Act. The good news is that we have more people covered today, but they continue to use the benefits in the same way the rest of us did in the past. So now we’re approaching 20 to 22 percent of GDP (health care spending). We need to bend this cost curve through education, better consumption and understanding of how to best use (insurance) benefits. If we all continue to use it the way we’ve always used it, it’s unsustainable.

I think what employers have learned is that if you don’t have skin in the game, you probably don’t care as much about how your money is spent. (An employee who pays nothing out of their salary for health insurance premiums) or has a very low deductible and really doesn’t hit your pocket very often (starts to become desensitized to the cost of care and the purchases you have to make today).

Some of the historical drivers would be the way we pay for services. It is a fee-for-service disease model rather than a preventive health care model. Each (medical) code has a cost. The more codes you enter, the more it costs you. There is a certain amount that is defensive medicine.

(Another) piece is that you have companies now moving to the minimum essential benefit programs associated with the Affordable Care Act. A small group employer is not required by law to offer (but) transition to the minimum essential benefits covered by the new plans (offered) under the Affordable Care Act. So that will push prices up a bit.

If you are thinking about being able to include strategic partners in a plan that can provide a cost benefit to the employer and cost-shared employee, it includes some of these delivery models. Consider patient-centered medical homes and ACOs (accountable care organizations). If you have organizations that are strategic partners working with you, you can translate that into a lower cost to an employer and you still get the benefit, but then you might be able to bend the cost curve a little bit.

It is not sustainable. I think a lot is about our usage pattern. Firstly, it has to do with the lifestyle we live. It’s about what we chose to do. At first glance, it can be difficult to tell the difference between these networks. Why choose BlueSelect over BlueOptions or BlueCare? Is one network better than the rest?

Well, I did some serious searching and asking, and here’s what I learned about Florida Blue’s three main design options.

BlueSelect, BlueCare and BlueOptions are essentially aliases for the type of network you’re signing up for. Here are the network breakdowns for each plan:

Essentially, all you need to know about a network is that it is a group of doctors, hospitals and other health care providers that have agreed to negotiate reduced fees through Florida Blue. That means you’ll typically pay less when you visit providers within your plan’s network.

This would probably be a good time to bring up the fact that CVS is leaving the Florida Blue pharmacy network for ACA plans in 2017. If you use a CVS, Target, or Navarro pharmacy and have an ACA compliant plan, now is the time to start looking for other options.

Now that we have all this figured out, we will move on to the next important step which is to find out if the doctors, specialists and hospitals you are using are in any of these networks.

This is where the magic happens. Since I have taken the time to add my providers to my list, I can now see which of my providers are in network with BlueSelect, BlueCare and BlueOptions.

This simple step has already made my network choice easier as I now know that I would not want to choose BlueSelect since my primary care doctor is not in network.

You still have to try to find the exact plan that fits your needs based on things like deductibles, deductibles, and co-pays, but at least you’ve cut back a little.
